Florida requirement for the identified preparer:
We understand the particulars of the Florida recording requirement for the identified preparer have been enforced inconsistently over time across Florida's counties.
The options available through GoDocs are:
- For Florida recordation, you can add a natural person to the "Prepared By" section of the document, per document, post-delivery; internally, or possibly with the title or escrow company.
- If you provide one name, we can include that name as an attention name with all recorded documents (not just Florida); this is understood to satisfy the Florida recordation issue regarding a natural person identified as a prepared party.
- If you had a specific request in mind, we would review that idea for coding as well.
Please note that of the first two options above, the option to add the name post-delivery from GoDocs is the most flexible solution used by our customers.

Florida Insurance Anti-Coercion Statement
If included, the Florida Insurance Anti-Coercion Statement has a blank line for the name of the insurance company, if the Lender has it.
If the name is not readily available, it will leave a blank line in the form for the borrower to complete at signing.
If you do have the name of the insurance company then please enter it into the field so it will populate into the document.
Florida Affidavit: Each entity or individual borrower/buyer should sign this Florida affidavit. If the borrower is an LLC entity (or other entity type) with two signers for that entity, each signer should sign the Florida affidavit.
Florida statues concerning foreign ownership
The check boxes are related to the Florida statues. The lender may want to seek guidance from a real estate professional in Florida; the title company may be able to offer assistance.
It is at lender's discretion to include neither, both or either of: (i) Affidavit for Individuals with Foreign Interests/Affidavit for an Entity with Foreign Interests (ii) or the Notice: Florida Now Restricts the Sale of Property to Foreign Principals. These forms are stand-alone documents and may be ordered to be used or discarded in the loan package at the lender's discretion. This is information that pertains to a specific property, owner, borrower or other particulars, and GoDocs otherwise does not would not have this information. You may want to work with the title company as they may have more information regarding the property.
As noted, GoDocs highly recommends the lender seek advice of counsel to determine whether such an affidavit is required.
